Data Protection Policy: Williams Physiotherapy

We take collection and safe storage of your personal information very seriously. The below policy applies to Williams Physiotherapy on behalf of the Williams Physiotherapy Group. It includes the collection, storage and processing of personal data under the Data Protection Act (DPA) 1998 and the General Data Protection Regulations (GDPR) 2018.
What information do we collect?

At time of booking we collect name, date of birth and contact number. During the initial assessment information related to your health including medical history, medication, details of presenting problem, investigations, GP details and other relevant information.

Why do we collect this data?
​
Contact data is needed to track appointments and relay any last minute changes if needed. Personal data is needed to appropriately assess and diagnose your presenting condition. It is a requirement under our professional standards to both assess and maintain these accurate records.
How is the data stored?

Whether you book online or are booked in by one of our team, your information is stored on a secure online server with password protected access only to authorised clinicians working for Williams Physiotherapy. We never share your information with any third party and will only discuss your case with qualified clinicians such as your GP if needed and with your consent. All of your treatment records will also be held on the same encrypted and password protected online server.
How long do you keep my records and information?

As part of GDPR we keep your data securely for 8 years where at that time the records will be safely deleted.
Am I going to be added to an email mailing list or cold called?

No. We take your email address solely for sending payment invoices and if we need to send you information regarding your treatment such as exercises. We will never spam you will sales emails. We will only contact you via telephone in response to a query from you, to change a booked appointment, or as a courtesy call to see how you are following a treatment session.

How you can access copies of your notes.
​
You have the right to see all the information Williams Physiotherapy holds about you including treatment notes. Any inaccuracies in the notes can be amended at your request also. To request a copy of your notes we need the request in writing and you are welcome to email the team at williamsphysiotherapygroup@gmail.com or write to Williams Physiotherapy, 22 Nicholas Street, Chester, CH1 2NX. To cover cost of administrative duties gathering this data we would charge a £15 administration fee.
Third Party Providers.

After thorough review of the best exercise applications I decided on Physitrack. I will create the bespoke treatment programme and it will be created on the Physitrack application. The only information they hold about you is your name, email address, and year of birth only. This data protection policy applies to Williams Physiotherapy clinic duties only.
